- Government issued ID and Address proff of Guests/ Children and Guardian
- Previous Medical Summary
- Current Medical Prescriptions
- Recent Lab Reports
- Covid Negative Test Report
- Covid Vaccination (Both Doses) Certificate
- 1 Minute long video showing the Senior Citizen walking
Guidelines
Rules & regulations at Peace Home for Senior Citizens
- Overnight Guests Not Allowed
- Repair Costs for Property Damage will be Deducted from Deposit
- Change of contact information of Guardian should be Notified within 24 hours
- Absolutely NO INTOXICANTS ALLOWED inside the premises
- Right to Admission and Continuance of Stay reserved
- Exiting the Premises with Local Guardian or Attendant (Provided at Extra Cost) will be allowed
- Any and All Medical Expenses are to be borne by the Resident/Guardian
